Arriving

I agree to arrive to my appointment at or 5-10 prior to the scheduled time. If I know I will be arriving a few minutes late, I will call or send a message to inform the practitioner of my circumstances. If I am more that ten minutes late I may not be able to receive the full amount of time that was scheduled for the appointment. In any of these cases, I am still responsible for the full payment.

 

Cancellations

If I need to cancel my appointment, I agree to cancel at least twenty-four hours in advance. If I have a late cancellation or I do not show up for my scheduled appointment, I agree to pay the no show/late cancellation fee. The fee is always 50% of the full amount of the scheduled appointment. This fee is separate from any packages or discounts. This is invoiced to my email and is to be paid at the time it is received. If for any reason this is not possible, I will pay at the time of my next appointment. 

Payment

I am responsible as the patient/patient’s guardian for full payment prior to or at the time of service, this includes emailed invoices. 

 

Misconduct

I understand that this is a strictly therapeutic, inclusive, and safe environment. Any inappropriate behaviors, language or sexual misconduct will not be tolerated. Should any sexual misconduct arise, the session will be immediately terminated and the appropriate protocol will be followed.

I understand that the practitioner does not diagnose illness or disease and does not prescribe medical treatment. I have stated all my known physical conditions and medications, and I will keep the massage therapist updated on any changes.

 

Client confidentiality is maintained at all times.
